Lower back pain is one of the leading causes of disability worldwide. For some people it is a dull ache after a long day; for others it is sharp, burning pain shooting down one leg, the classic sign of sciatica. Either way, it affects work, sleep, mood and independence.
At Greenwich Ayurveda Hospital, Kottakkal, we see many patients with disc problems, sciatica and chronic back pain. Most of them respond well to conservative, non-surgical care that combines Kerala Ayurvedic therapies with targeted physiotherapy.
Common Causes of Lower Back Pain
- Muscle strain from lifting, sudden movements or poor posture
- Lumbar disc bulge or herniation (slip disc), which can press on nerve roots
- Degenerative disc disease and lumbar spondylosis
- Lumbar canal stenosis, a narrowing of the spinal canal
- Sacroiliac joint dysfunction
- Sedentary lifestyle, weak core muscles and excess weight
What Is Sciatica?
The sciatic nerve runs from the lower back through the buttock and down the back of each leg. When a nerve root that forms it is compressed or irritated, often by a herniated disc, pain radiates along its path. Symptoms include:
- Pain from the lower back or buttock down the leg, sometimes to the foot
- Numbness, tingling or a burning sensation
- Weakness in the leg or foot
- Pain that worsens on sitting, coughing or sneezing
Red flags: seek urgent medical care
Go to a hospital immediately if back pain comes with loss of bladder or bowel control, numbness around the groin or inner thighs, rapidly worsening leg weakness, or fever with severe back pain. These can signal serious conditions such as cauda equina syndrome that need emergency treatment.
How Ayurveda Understands Back Pain and Sciatica
Ayurveda describes sciatica as Gridhrasi, named after the vulture (Gridhra) because of the way the pain changes the patient’s gait. Lower back pain is described under Kati Shoola and Katigraha. Both are primarily Vata disorders, sometimes with Kapha involvement causing heaviness and stiffness.
Treatment aims to:
- Pacify Vata and relieve pain
- Reduce inflammation and nerve irritation
- Nourish the discs, joints and supporting tissues
- Strengthen the muscles so that the problem does not return
Key Ayurvedic Treatments for Back Pain and Sciatica
Kati Vasti
A ring of black gram dough is placed over the painful area of the lower back and filled with warm medicated oil, which is held there for 30 to 45 minutes and rewarmed periodically. Kati Vasti is one of the most effective local therapies for lumbar disc problems, spondylosis and chronic back pain. Learn more about our Kati Vasti treatment.
Kizhi (herbal bolus therapy)
Warm herbal boluses are applied to the back and legs. Elakizhi is commonly used for pain with muscle spasm, while Podikizhi may be chosen when stiffness and heaviness dominate. Read our guide to choosing the right Kizhi therapy.
Abhyanga and Swedana
Oil massage followed by herbal steam relaxes tight back and leg muscles and improves circulation. See Abhyangam.
Vasti (medicated enema)
Vasti is regarded as the most important treatment for Vata disorders. A planned course of oil and decoction enemas is often central to treating chronic sciatica and degenerative spine problems. It is usually part of a Panchakarma programme.
Pizhichil
For chronic and widespread pain with stiffness, Pizhichil provides deep oleation and warmth.
Internal medicines
Classical herbal decoctions, tablets and medicated oils are prescribed to support the external therapies and continue healing at home.
Physiotherapy: Building a Stronger Back
Our physiotherapy team works alongside the Ayurvedic physicians on:
- Core strengthening and spinal stabilisation exercises
- Nerve mobility (neural gliding) exercises for sciatica, when appropriate
- Hamstring and hip flexibility
- Posture and lifting technique education
- A personalised home exercise programme
Simple Habits to Protect Your Back
- Avoid sitting continuously for more than 30 to 40 minutes. Stand, stretch and walk briefly.
- Lift with your legs, not your back. Keep loads close to your body and avoid twisting while lifting.
- Choose a medium-firm mattress that supports the natural curve of your spine.
- Keep your lower back warm and avoid cold drafts.
- Stay active. Gentle walking is usually better than prolonged bed rest.
- Maintain a healthy weight to reduce load on the spine.
- Eat warm, freshly cooked food and avoid constipation, which aggravates Vata.
How Long Does Treatment Take?
Acute back pain may settle with 7 to 14 days of therapy. Chronic sciatica and disc-related pain usually benefit from 14 to 21 days, followed by home medicines and exercises. Please bring any MRI or X-ray reports to your consultation.
Do You Really Need Surgery?
Most people with a slip disc or sciatica improve without surgery. Surgery is usually considered for severe or progressive nerve compression, red-flag symptoms or pain that does not respond to adequate conservative treatment. We will always advise you honestly if a surgical opinion is needed.
